Why Is My Cat Peeing Outside the Litter Box?

By Marta Lista DVM

Finding urine outside the litter box can be frustrating and sometimes a little heartbreaking. We know it can feel like your cat is acting out, but more often than not, they’re trying to tell you something.

At Trail Animal Hospital, we understand how upsetting this behavior can be. The good news? There are usually clear reasons behind it and many can be fixed with the right help.

Common Reasons Cats Stop Using the Litter Box

Cats are naturally clean creatures, so changes in bathroom habits are often a sign that something’s off. Here are some of the most common reasons:

  • Medical issues Urinary tract infections (UTIs), bladder stones, diabetes, kidney disease, or arthritis can all make it painful or difficult for your cat to urinate normally.
  • Litter box problems The box might be dirty, too small, hard to reach, or located in a noisy or stressful spot.
  • Stress or anxiety Changes in the household new pets, new baby, a move, even a new litter can stress your cat out and lead to inappropriate urination.
  • Territorial marking Especially in unneutered males or in multi-cat homes, urinating outside the box can be a way of marking territory.
  • Negative associations If your cat had a painful experience while using the box (like straining to pee with a UTI), they might start avoiding it altogether.
When Should You Be Concerned?

Any time your cat changes their bathroom habits, it’s a good idea to check in with your vet—especially if the issue came on suddenly.

Call your us if your cat:

  • Is straining to urinate or crying in the box
  • Is urinating in small amounts or frequently
  • Has blood in their urine
  • Is lethargic, not eating, or hiding more than usual
  • Is older or has a history of urinary problems

Catching urinary issues early can prevent bigger complications some conditions can become life-threatening if left untreated.

At Trail Animal Hospital, we’re committed to treating your pets like family. If you’re ever unsure, we’re just a phone call away ready to listen, guide you, and provide the care your cat needs to feel better.
